1. Personal illness;
2. Illness of immediate family member;
3. Death in the family;
4. Extreme weather conditions;
5. Religious observances; or
6. Circumstances which in the judgment of the principal create emergencies over which the student has no control.
Students are required to submit a note for absences to the office within 2 days of returning to school. Notes must be turned into the front office. Students with excessive absences may be required to have a note from a doctor before further absences will be excused.
Please print out the PDF below and complete for a note to excuse absences. This completed note should be turned into the office when the student returns to school.
Parents can submit the absence note form using the official Bon Lin Middle School app, and clicking on the Forms tile. Pictures of doctor's notes can be uploaded directly into the app. Parents can also use the link below to visit the web version of our App and submit an absence note.
